Specifications include, but are not limited to: 1. Mail or email applicable number of Initial Application Packets to applicants on the Waitlist to include: a. Required signatory forms for adult household members; b. List of required documents for income verification; and c. List of required documents for proof of identity. 2. Schedule and conduct intake interview to determine applicant eligibility. 3. Obtain required household income and asset verification documentations as reported on the applicant’s Personal Declaration Form to include without limitation paystubs and bank statements. 4. Obtain required proof of identify documentations for applicant and household members to include birth certificates, picture identification, passports, social security cards. 5. Send notice(s) to applicants for missing information/documentation, such as income, asset, etc. to determine eligibility: a. First notice shall require a 10-business day turnaround from the applicant to submit the requested documents. b. A second notice shall sent to the applicant to follow up on outstanding documents requested from the first notice. c. Failure to submit the required documents by the second notice due date may result in non-eligibility determination.
