Ticket Book Measurements: • Overall Size - 4.25” X 9.0” • Trim Size - 4.25" x 8.0" • Tabs - ½” top and ½” bottom Ticket Book Contents: • Cover (no printing) - 150# tag, manila, or equivalent, NO PERFORATIONS. • Wrap around cover - 150# tag, manila, or equivalent, print on spine of book matching starting alpha/numeric number of the first CHP 215 form in ticket book (AA #####), form number (CHP 215), and quantity (25 Sets), NO PERFORATIONS. • ASSIGNMENT OF NOTICE TO APPEAR (CHP 215A). First form in book, NO PERFORATIONS. • NOTICE TO APPEAR (CHP 215). Twenty-five (25) alpha/numeric numbered sets, PERFORATIONS. • Side stitch on the ½” stub, 2 ½” wide 20/24 gage stitches centered between two (2) drilled ¼” holes, 2 ¾” center-to-center. • Binding board will be .018 chipboard to be stitched into the back of the booklet. Binding board will be included in the cost. Description of Form CHP 215A, ASSIGNMENT OF NOTICE TO APPEAR: • First form in ticket book. • Print in black and the alpha/numeric numbering must match the first alpha/numeric number in the series of each booklet, no back printing. • Stock should be 20# pink bond, 4.25"x 8.0", plus ½” tab from left edge and no tab/perforation from unbound edge.
