Specifications include, but are not limited to: Work with AHS leadership to develop and implement Team Based Care 2.0 (TBC 2.0) Learning Collaboratives by District (there are 12 districts in total)1. TBC 2.0 Learning Collaboratives will aim to promote structure and consistency in communities and highlight the value of working together and creating a culture of care. They will also highlight need areas and help inform future policies and investments in payment reform (e.g., where are gaps where we need payment mechanisms to reimburse providers fairly and consistently for doing this work & IT/Data Sharing, etc.). To support the TBC 2.0 Learning Collaboratives, the Successful Bidder will: • Engage community leadership to discuss how best to revitalize team-based care approaches in their respective local ecosystems. • Work within each learning collaborative to help communities strategize and identify what data will help identify community members who may benefit from team-based care and what data to track to know if this initiative is making a difference in their community. • Identify and track how team-based care has increased collaboration and efficiencies among providers, increased equity in healthcare services provision, and decreased duplication and demand for crisis services.
