Specifications include, but are not limited to: Services to include but not limited to: • Picking up the deceased in a timely manner • Documenting the coordination of arrangements with the family (or making efforts to find the next-of-kin) • Identifying alternate sources of funding • Obtaining any needed releases • Completing all necessary paperwork in an accurate and timely fashion including: o the completed Salt Lake County Health Department Mortuary Application for Reimbursement which must be submitted to SLCoHD within one or two weeks or receiving remains as outlined within the Scope above; o (if applicable) the completed Salt Lake County Health Department Indigent Cremation application which must be submitted to SLCoHD within one or two weeks of receiving remains as outlined within the Scope above; o the death certificates; o submit the completed indigent application(s) and an official invoice to SLCoHD; and (the invoice must indicate the name of the deceased, the date of service, evidence of approval, and the amount due) o any other essential documentation as determined by SLCoHD • Cremating the remains in accordance with industry standards • Work with SLCoHD to successfully resolve any issues or complaints that next of kin may express.
