Specifications include, but are not limited to: QUANTITY: 1) 3,333 booklets, each containing 30 numbered decals and 30 matching-numbered inspection reports, for a total of 99,990 decal/report sets. 2) No overrun. Up to 1% underrun allowed. 3) Please also quote pricing and lead time for reorder of additional booklets after delivery of the initial order, as requested on the “Vendor’s Bid Proposal” page following these specs. CONSTRUCTION: 1) This is a 3” x 4-7/8” booklet bound at the head by two heavy-duty staples. 2) Each booklet shall be made up of the following parts, listed in order from top to bottom: A) A top cover; B) One sheet of 10 inspection decals with serial numbers, perforated for individual tearoff; C) Ten leaves of paper inspection reports, each with a serial number corresponding to one of the detachable stickers on the 1st decal sheet; D) Another sheet of 10 inspection decals with serial numbers; E) Another ten leaves of paper inspection reports with serial numbers corresponding to decals on the 2nd decal sheet; D) A third sheet of 10 inspection decals with serial numbers; E) Ten more leaves of paper inspection reports with serial numbers corresponding to decals on the 3rd decal sheet; F) A bottom cover 3) Shown below are reduced-size images of the components from a previous year. This year’s will be the same, but updated in text and decal color. 4) Top cover, bottom cover, and all interior decal and inspection leaves shall be 3" x 4-7/8". 5) All interior pages (the decal sheets and the inspector’s reports) shall be perforated to create a ½” binding stub, while the top and bottom cover leaves remain unperforated. Staples, stub perforations and printing must be correctly positioned so that all staples are completely in the stub area and all printing is below the stub perforation. 6) The decal sheets shall have ten (10) individual stickers of 1-1/2" x 7/8" size, arranged two abreast, five rows deep on the sheet, plus the ½” binding stub. Perforations between all stickers and the binding stub will permit tear-off of each sticker individually, complete with liner.
