Specifications include, but are not limited to: 1. A package of tools designed to screen, assess, and assist with support planning, measure outcomes to improve the lives of caregivers—including dementia caregivers—and their care recipients; provide a package of ongoing training for state users; provide evidence of effectiveness and quality; accommodate multiple cultural caregiver characteristics and needs. 2. A caregiver support program. Bidding organizations should provide the following information about the caregiver support program that the State would be implementing if the bidding organization is selected: a) Theory of caregiving driving the approach b) Whether the program is intended to be provided telephonically, in person, or via another mode. c) Whether the program has been designed to meet the needs of caregivers of people with dementia. d) Screening and assessment process, including the outcomes of each of these processes. e) Support planning, including whether the program results in any algorithms or other criteria that helps guide the assignment of support in a manner that is tailored to the caregivers’ needs and preferences. f) Ongoing support as part of the program, including requirements for providing support and mechanisms for tracking that support. g) Training infrastructure, including required training and how training is implemented. h) Data points and types of data collected, including data that can be used as process or outcome measures. i) Automation, including whether use of the automation is required or whether the tools can be incorporated into other automation that Vermont might use and the ability to share and receive data from other systems in a manner that complies with HIPAA requirements.
