Specifications include, but are not limited to: The New Hampshire Department of Health and Human Services, Division of Public Health Services (“Department”) is seeking responses to this Request for Applications (solicitation)from qualified Vendors to assess the readiness of the Department to complete the Public Health Accreditation Board (PHAB) state health department accreditation process, and to evaluate potential benefits and challenges to achieving accreditation. 2.1.1. The selected Vendor must assist the Department in assessing readiness for accreditation by addressing the following PHAB domains: 2.1.1.1. Assessing and monitoring population health status, factors that influence health, and community needs and assets; 2.1.1.2. Investigating, diagnosing, and addressing health problems and hazards affecting the population; 2.1.1.3. Communicating effectively to inform and educate people about health, factors that influence health, and how to improve health; 2.1.1.4. Strengthening, supporting, and mobilizing communities and partnerships to improve health; 2.1.1.5. Creating, championing, and implementing policies, plans, and laws that impact health; 2.1.1.6. Utilizing legal and regulatory actions designed to improve and protect the public’s health; 2.1.1.7. Contributing to an effective system that enables equitable access to the individual services and care needed to be healthy; 2.1.1.8. Building and supporting a diverse and skilled public health workforce; 2.1.1.9. Improving and innovating public health functions through ongoing evaluation, research, and continuous quality improvement; and 2.1.1.10. Building and maintaining a strong organizational infrastructure for public health.
