Specifications include, but are not limited to: 1. Comply with all OSHA Hazwoper regulations throughout each phase of the project. This includes but not limited to Level D PPE. Sleeveless shirts, shorts, and sweatpants are not permitted. Also, no food, drinks or smoking shall be allowed in the work area. 2. Have an "USEPA/City approved" Health and Safety Plan (HASP) (consistent with 29 CFR 1910 & 1926) and Storm Water Run-off Plan (SWRP) on file with the City of Omaha. 3. Have a file with the City of Omaha; current 40 hour Hazwoper certificates for workers and supervisors and XRF training certificates for associates that will be operating an XRF. Additionally, supervisors must have a current 8 hour Hazwoper supervisor certificate. 4. Retain all insurance & certifications in "current" status during the tenure of the project. 5. Prior to remediation; request underground utility location, acquire all applicable permits and submit utilities locate confirmation, materials (soil, sod & mulch) testing confirmation and Toxic Characteristic Leaching Procedure results documentation (TCLP) to City staff for approval. 6. Utilize an NLLAP approved Laboratory for all materials analysis. (Soil organics can be analyzed by Midwest Labs) 7. Contact the PROPERTY OWNER at least three (3) days prior to performing any remediation operations. 1f contact cannot be made. City staff may assist in making contact. Projects cannot begin without making contact. 8. Provide a remediation schedule to the City staff prior to performing remediation operations on a daily or weekly basis. 9. The Contractor's Project Manager must be on-site (or at the site within 30 min, if requested by the City. 10. Request an inspection by City at the end of each phase of each project before beginning the next phase (excavation, back-fill and sod installation) Record all lnsitu readings on the field sheet for City review.
