Specifications include, but are not limited to: STATE is soliciting proposals for an entity to perform assessments for clients with Intellectual or DD utilizing the SIS and the ICAP assessments. The initial assessment shall be completed within 90 days of the successful offeror being notified or at the time there are sufficient qualified responders of the client beginning a qualified service. A reassessment of client needs using the standardized assessment tool must be completed every 36 months for a client age 16 years or older, or every 12 months for a client under age 16 years. Out of sequence assessments may be required if the client experiences life changing events. In those cases, the appropriate assessment must be completed within 45 days of the successful offeror being notified. In the event an assessment is unable to be completed within these timelines, the successful offeror must work with STATE to remedy the delay. The successful offeror must: Identify its familiarity with the SIS and ICAP requirements and assure that these requirements are followed. Begin assessments at the start of the contract, which will require successful offeror’s staff to meet the required training per assessment guidelines. Schedule and complete assessments timely and track the timelines for assessments. Assist with policy and procedure developments, updates, or enhancements. Assist state with ad-hoc reviews and trend analysis, as needed. Communicate concerns and other essential information to STATE. Request and collect any necessary pre-assessment information. Complete and collect assessment protocol checklists. Provide monthly reports that include the following: Number of assessments completed each month. Experience Survey results. Summary of results and number of provider checklists received. Summary of results and number of protocol checklists completed.
