Specifications include, but are not limited to: ☐ Follow state reporting and quality standards and guidelines. ☐ Ensure all B-5 funded staff (home visitor(s) and supervisor(s)) complete all required training. ☐ Ensure all B-5 funded staff (home visitor(s) and supervisor(s)) complete all trainings required by DPHHS, including but not limited to trainings on the required screening/assessment tools, data collection, and the Maternal and Early Childhood Home Visiting (MTmechv) data system. ☐ By 30 days after the contract start date, set a final caseload agreement with B-5 reflecting number of clients the contractor expects to serve when fully staffed. ☐ Develop a client consent form to meet the organizational and DPHHS requirements and submit it to B-5 for approval by 30 days after the contract start date (templates will be provided during the contracting process). ☐ Provide visits to clients on a schedule consistent with the pilot requirements throughout the client’s enrollment in the program which meets agreed upon caseload requirements. ☐ Conduct screenings and assessments with clients using the tools and schedule identified by DPHHS and ensure screening results are retained in the client’s record. ☐ Participate in required, regular conference calls, coaching calls, meetings, and site visits as requested by DPHHS. ☐ Participate in evaluation and Continuous Quality Improvement (CQI) activities as a home visiting site and with DPHHS as requested and required by B-5. ☐ Submit Monthly Implementation Site Reports (MISRs) to B-5. ☐ Submit timely accurate, complete, reliable monthly expenditure reports to the DPHHS.
