Specifications include, but are not limited to: 1. A visual inspection shall be made of the complete aerial device and other areas which may affect unit safety, stability, and performance. 2. Check for proper operation such as hand chain wheels, chain attachments, clevises, yokes, suspension bolts, shafts, gears, bearings, pins, rollers locking and clamping devices for evidence of wear, corrosion, cracks and/or distortion 3. Hoist braking system for proper operation 4. Check for worn, glazed or/oil contaminated friction discs, worn pawls, cams, or ratchet 5. Check for corrected, stretched or broken pawl springs in brake mechanism 6. Check connections of load chain or wire ropes for evidence of wear, corrosion, cracks, damage, and distortion 7. Check labels on pendant control stations on electric and air powered hoist for legibility 8. Check oil levels and lubrication 9. Check hoist and hoist mounting for evidence of missing items 10. Inspect chain/ rope in accordance with Section 16-2.5.1, 16-2.4.1, and 16-2.6.2 of ASME B30.16B,1995 11. Reports of inspections/ tests, indicating make model of device tested and test results, shall be furnished to the requesting INDOT Facilities Manager at the time the test was performed. 12. Inspection stickers indicating the date of inspection/test and the inspector identification shall be affixed to the equipment upon completion of inspection and testing 13. The successful contractor shall make available all certification forms of each person accomplishing test procedures. All testing technicians are to be SNT-TC-1A certified. 14. The successful contractor shall be required to furnish and have exclusive control of all personnel, equipment, tools, materials, and supplies required to provide services as set forth herein.
