Specifications include, but are not limited to: 18.1. Complete 14-day health appraisals including: 18.1.1. Medical history 18.1.2. Current medications 18.1.3. Recording of height, weight and vital signs 18.1.4. Medical examination with evaluation of basic medical, mental and dental status. 18.1.5. Review positive findings with on-call medical provider via telephone, text, fax, email or telemedicine. 18.1.6. Mental, dental and mental health screening is performed by qualified health-care personnel on all residents upon arrival at the facility. The screening includes: Inquiry into: 1. Current illness and health problems, including venereal diseases and other infectious diseases; 2. Dental problems; Mental health problems, including suicide attempts or ideation; 3. Use of alcohol and other drugs, which types of drugs used, mode of use, amounts used, frequency of use, date or time of last use, and a history of problems that may have occurred after ceasing use; Observation of: 1. Behavior which includes states of consciousness, mental status, appearance, conduct, tremor and sweating; 2. Body deformities, ease of movement, and so forth; 3. Condition of skin, including trauma markings, bruises, lesions, jaundice, rashes and infestation, and needle marks or othe indications of drug use.
