Specifications include, but are not limited to: 1. School‐Based Health Centers (SBHCs) function within state and federal laws and established standards of practice, including standing orders or collaborative agreements established by the medical contractor(s). Administrative oversight of SBHCs is provided by the Division of Public Health, Department of Health and Social Services and the Colonial School District. As such, the school‐based health center and its on‐site management team are accountable to the Division’s quality assurance plan and regular site visit monitoring review as well as Colonial District Office Administration. 2. The School‐Based Health Center (hub site) must be open for student services (physical and/or behavioral health) on all Colonial student days. The School-Based Health Center must be open on Professional Development days for the purposes of student services, participation in vendor professional development, and/or participation in school-based professional development. 3. Service hours for students will be in alignment with Colonial elementary student hours (i.e. 8:55am-3:35pm). 4. The School-Based Health Center (hub and spoke sites combined) is expected to schedule and complete at least 345 appointments per month during the school year. This is based on the assumption that each practitioner has 72 contact hours available each month and will maintain an 80% ‘fill rate’. Failure to meet the expected number of appointments for two consecutive quarters will result in a meeting with Colonial School District and the vendor to discuss the terms of the contract. Result of meeting may include reduction in compensation as a result of reduced services provided. a. This indicator may be revisited and re-negotiated annually based on a review of available data. 5. The medical sponsor is responsible for making an application to the DPH to be state recognized as a SBHC.
