Specifications include, but are not limited to: The South Dakota Department of Health (DOH) is seeking a partnership to create a medical respite program serving Pennington County, South Dakota that serves individuals experiencing homelessness as well as other individuals meeting care criteria. The program must be focused on decreasing inappropriate healthcare system utilization, providing infrastructure to support the community during system strains, improving community population health and well-being, providing cost savings, and ensuring job creation. 10 Offerors should define the care navigation services available for individuals experiencing homelessness within the medical respite program. Along with Describing what methods of the national care model will be used or not used. Key deliverables of this work include, but are not limited to; A. Hire personnel and pay their wages to establish the medical respite program. These positions may include a project manager, hospitalist/addiction medicine specialist, Advanced Practice Provider, Clinical Supervisor and Community health worker to stand up the newly established program. B. Purchase consumable supplies as needed to establish the medical respite program. C. Travel to other medical respite programs and training opportunities to establish the medical respite program. D. Provide emergency lodging for homeless individuals. Specify if this lodging is staffed by medical staff providing medical services e.g., evaluation, observation, supervision or any additional services. E. Implement a clinical decisions support system (CDSS) software for the medical respite program. Describe the functionality of this system; what will it do and how it brings value to the program. F. Train and employ community health workers to provide care navigation, transportation and support to program participants. G. The medical respite program will develop policies and procedures to become a sustainable Medicaidbilling program, which also qualifies as medical services. H. Submit monthly progress reports to the State to provide progress on each of the preceding deliverable areas which must- accompany monthly invoicing to provide accountability for contract deliverables.
