Scanners: Must scan accurately on Department of Taxes Scanners. 2.10. Ordering: Forms to be ordered 100,000 to 200,000 at a time as needed. 2.11. Delivery: Forms to be delivered to Vermont Department of Taxes, 133 State Street, Montpelier, VT 05633-1401. Inside delivery is required. Loading dock is available. Short bed truck only.
