Specifications include, but are not limited to: Provider service must satisfy licensing requirements under 902 KAR 20:300, Section 15, paragraph 9 as those requirements apply to long-term care facilities. Services are required to be in full compliance in accordance with all applicable Federal, State, and Local guidelines. Provider must be qualified to provide services in the Commonwealth of Kentucky. The provider of services shall be licensed to practice in the Commonwealth of Kentucky. Provider of mobile X-Ray services must not be an excluded Medicare/Medicaid provider. Make all X-Ray services available to all residents without regard to race, color, creed, national origin, age, sex, religion, or handicap. Provide orientation in-services to JEBWKVC directly related to X-Ray Services such as completing the requisitions but not limited to. The orientation in-services to staff at JEBWKVC will be at their convenience and at no cost to the facility.
