Specifications include, but are not limited to: A. The Contractor shall provide a half (1/2) page vertical, four-color professional journal print advertisement subscription (Exhibit A- Attachment 1) for 24 months upon execution of this contract. The advertisement dimensions will be 5.0417” Wide x 13.75” high. Revisions to dimensions and/or layout of the print advertisement will not necessitate an amendment to the agreement. B. Online Job Opportunity: Job posting, banner ads. Advertisement to remain posted online for entire contract at no additional cost. C. Contractor shall provide a listing of all designated employees to DSH with their vaccination status. Listing shall include dosages completed and exemptions on file. Listing shall be updated as needed and provided to the DSH Contract Manager upon request. D. Contractor, their personnel, subcontractors, and anyone else affiliated with this Agreement shall not engage in, and shall report all instances of, any activity that would constitute "Workplace Violence" as defined in the applicable DSH Administrative Letter, which can be provided upon request. Failure to comply with this provision by Contractor, their personnel, subcontractors, and anyone else affiliated with this Agreement shall be deemed a material breach of this Agreement.
