Services shall include, but are not limited to, the following: • OCCUPATIONAL HEALTH: Pre-placement examinations for all classes of employees, Annual physical examinations for certain positions (Police, EMS, Sheriffs), Biennial physical examinations for certain jobs (Crossing Guards), DOT physicals for various classes, Fitness for duty for all classes of employees, return to work examinations for all classes of employees, and disability determinations for all classes of employees. Evaluations in specific areas such as musculoskeletal, spine, head trauma, etc., must only be done under the supervision of a medical doctor specializing in occupational medicine, preferably with Board certification. • DIAGNOSTIC TESTING: Hearing, vision, pulmonary function, EKG, clinical lab services, Xray, MRI, CT, nuclear medicine, mammography, TB, and hepatitis. • DRUG SCREENING: Alcohol and controlled substance testing to include: o Post-accident and reasonable suspicion testing for all classes of employees. • On-site and off-site testing must be available. Testing during normal business hours with the ability to provide services at all other times (evening, weekends, holidays) upon request. o CDL testing for new and current employees; random testing of approximately 100 CDL personnel per year. NOTE: Providers must adhere to the U.S. Department of Transportation Federal Motor Carrier Safety Administrations Alcohol and Drug testing Rules to include certified technicians and medical review officer.
