Specifications include, but are not limited to: 1. Contractor shall provide all supervision, labor, materials, tools and equipment necessary to satisfactorily complete sanitary services maintenance, repairs upgrades and new install as detailed below and in accordance with Attachment D, Other Provisions, Small Scale Construction Services and Projects and Attachment E, Department of Forests, Parks and Recreation, Scope of Work (SOW), Request for Quote (RFQ). Scope of Work: a. At the request of the State, Contractor shall provide services on an emergency and nonemergency basis. i. Emergency work shall have an appropriate response time as determined by the State, including an immediate response. Response time will be determined by the State on a case-by-case basis, according to the imminent threat to personal safety of park visitors and staff and their personal property. b. This contract may be utilized for projects and services not to exceed $150,000.00. c. All Contractor work performed under this contract shall be planned and scheduled by Project Managers from the Department of Forests, Parks and Recreation (FPR). 2. When FPR has a need for services under this agreement, it will prepare a scope of work as detailed below: a. > $10,000.00 – for small projects and services under this threshold, FPR may request services via a phone call, email or a SOW-RFQ. Verbal approval is all that is required for services under this threshold. b. $10,001.00 to $50,000.00 - for projects and services at this threshold, FPR will prepare a SOW-RFQ form and will present it to one or more Contractor(s) under FPR contract. i. Contractor(s) shall then complete the SOW-RFQ form and submit to the Project Manager within the date and time established on the SOW-RFQ form. ii. Following evaluation of the SOW-RFQ, and when in the best interest of the State, FPR will sign and return the form which will serve as your notice to proceed. iii. FPR Project Managers will work with Contractor to review the approach, schedule and establish lines of communication for the project. c. $50,001.00 to $150,000.00 – for projects and services at this threshold, FPR will prepare a SOW-RFQ form and will present it to three or more Contractor(s) under FPR contract. i. Contractor(s) shall then complete the SOW-RFQ form and submit to the Project Manager within the date and time established on the SOW-RFQ form. ii. Following evaluation of the SOW-RFQ, and when in the best interest of the State, FPR will sign and return the form which will serve as your notice to proceed. iii. FPR Project Managers will work with Contractor to review the approach, schedule, and establish lines of communication for the project.
