Alameda County Health, Public Health Department (ACPHD) is seeking a qualified Bidder experienced with California Children's Services (CCS) program operations, to provide consultation, training, and support with program development and management of quality improvement activities for the CCS program in preparation of the Memorandum of Understanding (MOU) with Department of Health Care Services (DHCS). The selected Bidder will review the CCS programs current policies and procedures and develop a workplan for quality improvement initiatives, including evaluation of the CCS Enhanced Care Management program, improving medical home linkages for CCS clients, and implementing DHCS required monitoring and oversight activities.
