Specifications include, but are not limited to: A. The targeted pests include all pests such as, mites, earwigs, sow bugs, centipedes, millipedes, ants, spiders, cockroaches, scorpions, fleas, silverfish, and all other pests (excluding termites). Upon request, trapping for gophers, squirrels, mice and rats. B. The Contractor must provide the CHP on-site contact with material safety data sheets on any nonchemical or biological controls used in the treatment of pests that will be applied. C. License Requirements: One (1) Structural Pest Control Operator License Two (2) Qualified Applicator License (Category A and Category B) and Three (3) Structural Pest Control Board, Branch Two (2) License (See item 5, Specifications for details). D. Contractor agrees to use non-chemical or biological odorless or low odor type control. If this treatment is not effective, Contractor shall move to the use of Tier 3 (least hazardous) herbicides/insecticides and then progress to Tier 2 and then to Tier 1 if necessary, to manage pests. Contractor is responsible for adhering to this process and ensuring that they are using the most current tier-rated herbicides/insecticides. E. A report naming types and amounts of non-chemical/biological or tier-rated herbicides/insecticides used at the CHP locations must be submitted with each service, signed by the CHP on-site contact. F. Contractor shall perform services during the term of the contract to maintain pest control. Contractor shall provide report to the CHP on-site contact prior to and at the completion of each requested service. G. All quotations for additional services not covered under this agreement, including inspections for parasitic fly and vertebrate pests, will be provided to the CHP Facilities Coordinator. Funding for these abatement services will be outside of this contract. H. Contractor shall take all necessary precautions to protect service locations. Any damage caused by Contractor to state equipment, materials, or other priority shall be repaired or replaced by Contractor to the satisfaction of the state at no cost to the state. The state may, at its option, repair any damage and deduct from Contractor the cost thereof. Contractor shall remove and dispose of all debris created during performance of services.
