Specifications include, but are not limited to: Contractor shall establish, at a minimum, one (1) Transitions Clinic Program to provide culturally and linguistically responsive health and social care services (defined as medical, behavioral, dental, and social service needs) to justice-involved clients exiting San Diego County jails. As part of the program, Contractor shall hire Community Health Workers (CHWs), with lived experience of incarceration, to meet with justice-involved clients to build rapport and provide mentorship and guidance during their transitional phase prior to release, when possible, and after release from San Diego County jails. In this way, the CHWs will empower justiceinvolved clients to connect to needed health and social care services in the community upon reentry.
