The South Dakota Department of Health, Office of Child and Family Services, Home Visiting program is seeking proposals from partner organizations to provide evidence-based, voluntary home visiting services using the Parents as Teachers (PAT)® model for South Dakota families with children from birth to kindergarten entry. Services should be provided in selected, at-risk communities identified in the South Dakota needs assessment. Home visiting services are intended to be implemented at the community level as part of an integrated and coordinated system of maternal health and early childhood services. Programs will collect and report program data and implement a continuous quality improvement plan to demonstrate progress towards intended goals.
