Specifications include, but are not limited to: A. Onsite and/or facilitation of remote Biometric Screening B. Annual Health Risk Assessment Questionnaire (online platform) and detailed wellness reports for the organization as a whole and for each member (Please provide examples of each) C. Incentive design strategy for participation based and outcome-based programs D. Health Coaching for multiple conditions and behavioral health issues including tobacco cessation E. Web portal with capability to track onsite health and wellness activities (flexible enough to accommodate different events at different sites) F. Superior analytics of program engagement at the organization and member levels G. Ability to integrate with current benefit systems and accept data uploads from screening partners or other related vendors H. Standard and customized reports of participant data/activity readily available to program coordinator at all times I. Availability of mobile phone app or mobile website compatibility J. Ability to utilize UCA branded Communication Materials K. Wellness challenges and events tailored for the culture of the organization L. Reasonable Alternative Standard Management, it is preferred that disease management programs (COPD, Diabetes, CAS, Cancer Support, CHF, etc.) be included M. Attendance at UCA benefits fair and/or other campus events N. Provision and distribution of program activities and materials in languages other than English
