Specifications include, but are not limited to: A. Home Modifications Administration (Appendix B) DSAAPD is seeking a service provider in Delaware to select, coordinate, and manage contractors, among other related tasks, to support the successful construction of approved home modifications. DSAAPD anticipates completing approximately 140 home modifications. The grant funding allocated for this initiative supports the construction of the following: • Provision and installation of home mobility aids (i.e., wheelchair ramp, handrails for interior or exterior stairs or steps, grab bars, stair lifts of glides, Hoyer lifts or track systems, and emergency technology including lights and sirens). • Physical adaptations to the interior of an individual’s residence (i.e., widening of doorways or modification of bathroom facilities). B. Vehicular Modifications Administration (Appendix C) DSAAPD is seeking a service provider in Delaware to select, coordinate, and manage contractors, among other related tasks, to support the successful installation of approved vehicular modifications. DSAAPD anticipates completing approximately 4 vehicular modifications. The grant funding allocated for this initiative supports the installation of the following: • Provision and installation of vehicle modifications to support individuals and their caregivers (i.e., primary and secondary driving controls, ramps, lifts, wheelchair tie-downs and occupant restraint systems, and transfer seats). C. Home Inspection Services (Appendix D) DSAAPD is seeking a service provider in Delaware to conduct inspections of home modifications upon completion of the modification construction. The service provider will inspect the home modifications completed per the related Scope of Work to ensure that the construction is complete, safe for use, and meets relevant building code and laws. DSAAPD anticipates the need for approximately 140 home inspections.
