Department of Health, Family Health Services Division, WIC Services Branch is requesting bids from qualified independent (self-employed) nutrition assistant contractors, to perform virtual services to WIC participants located at our Hilo WIC clinic location. Please see attached job description of minimum requirements. Desirable Qualifications: * Available to work a minimum 24 hours per week; maximum 40 hours per week; * Able to set a work schedule between Monday - Friday; 7:45a - 4:30p HST (Hilo clinic office hours); * Capability to conduct virtual nutrition and breastfeeding education; * Previous WIC experience with food and nutrition program; * Preferred but not required nutrition assistant located on-site in Hilo; * Preferably familiar with HiWIC database. Please include resume and final transcripts with submission of bid. Web camera to perform virtual services (IT Requirement). Fill out the attached excel spreadsheet, include hours of availability (i.e. - 24 hrs; 40 hrs; etc), and submit with your bid. Include all taxes and fees, etc., in the hourly wage. Invoices will be submitted monthly, accompanied by supporting documentation time reports. This is an independent contractor position (self-employed), and not a State of Hawaii employee position. Upon award, Awardee will be required to submit current W9, sample invoice on letterhead, including remittance address. Option to extend two (2) additional three (3) month periods without any increase in hourly wage. Nutrition Assistant services are contingent upon availability of federal funds.
