Pathways Home, the City-County homelessness response plan, includes a team dedicated to building a Pathways to Housing list and assisting individuals in accessing housing and related supports through various community and public assistance programs. This team is seeking a software solution (mobile/web app) to support community outreach by County staff and community partners. The solution should enhance accessibility, data collection, validation efforts, and collaboration among different organizations while in the field. The Pathways to Housing list is a comprehensive record of approximately 200-300 individuals experiencing homelessness in the City of La Crosse, containing personally identifying information and approximately 50 various data points on each individual. The information collected is used in the following ways: • As a reference for community agencies to track the number of individuals experiencing homelessness, certain demographics, housing barriers, and needs. • To identify and monitor population trends, including inflows and outflows. • To inform the public of key figures related to homelessness via a data dashboard on the County website. • To guide strategies and interventions used to end homelessness in the City of La Crosse. The data collected includes sensitive personal information, and it is imperative that the solution complies with HIPAA standards to ensure the confidentiality and security of this data. The solution must also allow for monitoring individual access to ensure compliance and security. The intention of the RFI is to seek information from providers to gain knowledge about existing solutions, gather information about the feasibility of custom-built solutions, and obtain an idea of the costs involved. Once this information is obtained, the County will use it to decide whether to proceed with one of the options presented or to release a Request for Proposal (RFP).
