Specifications include, but are not limited to: provide early identification services for targeted families who are at risk for poor child health outcomes, poor child development, or child maltreatment to determine if they meet home visiting program eligibility criteria. Early identification services include screening women in birthing hospitals, developing sources for receiving referrals, and screening families from referrals to determine program eligibility. Eligible families are referred to home visiting programs within the Your ʻOhana Network for voluntary enrollment in home visiting services. Services include but are not limited to: maternal and child health, child maltreatment prevention, school readiness, domestic violence, family economic self-sufficiency, and coordination and referrals for other community resources and supports.
