The purpose of the project is to provide prescription medication and durable medical equipment for lowincome, uninsured adults in Sedgwick County through an existing coordinated system of healthcare services. The successful proposer(s) will develop a system to determine eligible service recipients based on 200% or lower of the current Federal Poverty Guidelines and residency verification, and link recipients to donated medical care through an existing coordinated network of healthcare providers. Specific outcomes could include access to prescriptions, durable medical equipment, and basic care and preventative doctor visits, to significant medical procedures. The services are not limited to medical needs and may include dental needs. The Request for Proposal should include a detailed description of the existing coordinated healthcare system access through leveraged or donated physician and dental services; including the number of participating physicians, hospital systems, dentists, pharmacies and other allied health care services, such as physical therapy and hospice care; and the experience detail, eligibility process, number of low-income patients served and all other pertinent data.
