Specifications include, but are not limited to: Pursuant to Chapter 103D-301, Hawaii Revised Statutes and Chapter 3-122-16, Subchapter 4.5, Hawaii Administrative Rules, "Source Selection for Federal Grant," the Department of Health ("DOH"), Chronic Disease Prevention and Health Promotion Division (“CDPHPD”) requests input and comments from current and prospective providers for the Centers for Disease Control and Prevention ("CDC") notice of funding opportunity ("NOFO") CDC-RFA-DP-23-0014: Racial and Ethnic Approaches to Community Health (“REACH”). This multi-component 5-year program aims to improve health, prevent chronic diseases, and reduce health disparities among racial and ethnic populations with the highest risk, or burden, of chronic disease, specifically among African American, Black, Hispanic, and Latino, Asian American, Native Hawaiian/Other Pacific Islander, American Indian, and Alaska Native populations. The project will support evidence-based, culturally tailored strategies at the local level among Native Hawaiian and Other Pacific Islanders (“NHOPI”) through prevention efforts that focus on interventions and activities for (1) nutrition, (2) physical activity, and (3) tobacco prevention and control policies.
