1. Furnish all labor, materials, equipment, transportation, permits, etc. — to provide medical waste service for Central Louisiana Supports and Services Center. 2. Agency will package and secure the appropriate materials in red bags supplied by vendor specifically designed for bio-medical waste and place in the vendor's boxes. 3. Vendor shall be responsible for picking up boxes of potentially infectious bio-medical waste from building's designated storage site and responsible for loading in to transportation vehicle. 4. Approximately three 28 gallon containers of regulated medical waste are to be picked up on a monthly schedule agreed upon by Central Louisiana Supports and Services Center and vendor. Other services include Hazardous Waste Incompatibles as needed, Hazardous Waste (D/U) bi-annual, RX destroyer product two cases annually, two gallons of controlled substance disposal annually. 5. Services are to be performed between 7:00am and 3:30pm Monday-Friday. 6. Contractor must be permitted as an infectious waste transporter by the Louisiana Department of Health and must furnish their permit number. 7. All infectious waste shall be transported to an approved "destruction of infectious waste" storage/treatment facility. 8. All infectious waste shall be transported in fully permitted and insured vehicles that are in compliance with all existing appropriate local, state, and federal regulations. Contractor will take responsibility of waste at the time that waste is fully loaded onto vehicle. 9. The agency may need additional red bags to be picked up. If the agency needs additional bags picked up they should be charged per individual bag.
