1. From the City's initial notice, the contractor will immediately provide a date and time for responding to the location for initial condition assessment. The contractor's timeframe must be with 48 hours of the City's initial notice. 2. The contractor will be required to set up an account under MyGov as a collaborator in order to access, process jobs, and submit documentation. Once the MyGov account is established, the contractor’s account can be accessed through the https://web.mygov.us/ website, by logging on using a mobile device or computer. 3. At a minimum, the information collected for each tree or forestry subject matter must include: a. Property Owner Name b. Parcel Identification Number and Relevant Information c. Address or Other Identifying Location: Clearly identify the location of each tree as required by the City’s request, including, but not limited to: i. Address ii. X and Y Coordinates in the specified format iii. Mobile Home Park Name d. Photos: The following types of before and/or after photos must be taken, along with any other photos that capture the property in violation: i. Overall View: Photos showing the general area and context of the tree or forestry matter within the property. ii. Close-Up Views: Detailed photos of the tree or forestry matter, highlighting any specific issues or conditions. iii. Condition of Surrounding Area: Photos of the area around the tree or forestry matter to show any impact or potential hazards. iv. Identification Photos: Clear images that include any identifying features or markers related to the tree or forestry matter. e. Condition: i. Good: A tree in good condition exhibits no major issues and is healthy. ii. Fair: A tree in fair condition has minor problems that may be addressable with time or corrective measures. iii. Poor: A tree in poor condition has significant issues that are unlikely to be resolved and may be irreparable. iv. Dead: A dead tree shows no signs of life and is beyond recovery. f. Primary Maintenance: The following maintenance needs may be assigned as a result of the condition assessment report by the Property Maintenance Unit Director: i. Removal: trees or forestry subject matter that are designated for removal will have defects that cannot be practically or cost effectively treated. The majority of trees in this category will have a large percentage of dead crown. ii. Pruning: this is the removal of one or more limbs, needed to reduce risk, provide clearance, and restore the conditional health of the tree and other forestry matter.
