The Madison Metropolitan School District (MMSD) is seeking a vendor to provide fluid milk for the National School Lunch Program, School Breakfast Program, After School Snack Program, and Summer Food Service Program (SFSP) district-wide. The contract is based on pricing provided by the distributor for the cost of processing and delivering fluid dairy sourced by the District. The District seeks a dairy distributor to provide milk coolers for and deliver milk to approximately 49 separate locations. The Vendor selected shall, no later than 7 days prior to the first deliveries, submit a proposed delivery and operational plan that includes the following: ❖ Schedule of deliveries by day and location; ❖ A roster of drivers, their assigned routes, and contact information including mobile phone numbers. Vendor will maintain an updated contact list as needed; ❖ Contact information for person(s) responsible for supply/inventory, delivery, quality/safety, equipment, and other logistical issues.
