A. Assist QACDOH with medical laboratory services for pap smears and colposcopiesincluding, but not limited to the following: a. The lab will provide all necessary supplies needed for the specimen to be collected and transported to the lab for testing, i.e bottles, caps, labels, security tape,chain of custody, and lab slips. All supplies, including containers, to be included in the cost. b. Laboratory will need to establish an account and bill our Family Planning program. Account number’s to be preprinted on lab slips. c. Laboratory will supply an insurance list and a listing of participating MCO’s. d. Laboratory must be willing to bill patient’s insurance, Medical Assistance Coverage or MCO, and bill the Queen Anne’s County Department of Health for those who have no coverage or coverage is denied. e. Laboratory must be willing to bill the State of Maryland’s Breast and Cervical Cancer Diagnosis and Treatment program for biopsies if the patient is in the program and has no insurance. f. Any denials should be billed to the Queen Anne’s County Department of Health, not the patient. g. Laboratory will provide a contact person for our registrar or a patient to call concerning questions on invoices. h. Laboratory records containing client information will be stored in a secured filing cabinet(s). Including in these records i.e., edit records, testing worksheet, data tape, chain of custody form, HIPAA forms, etc. Any data regarding patient information is to be confidential. Records to be stored for three years, in accordance with law regulations pertaining to storage of records. B. A Five (5) to Seven (7) day response time is required from receipt of specimen.
