1) Full Disclosure (First Time Referral): A test administered to confirm an Offender’s self-report regarding their sexual History Background Information to be included by P&P with referrals to Polygraph Examiner (if available and applicable). • Police report • Psychosexual evaluation • Recent violation report • Sexual history disclosure paperwork, if applicable. 2) Maintenance: A test administered to confirm an Offender’s compliance to supervision and treatment conditions, over a set period of time. Ideally, these exams are administered every six to nine months. Background Information to be included by P&P with referrals to Polygraph Examiner (if available and applicable). • Most recent polygraph examination report from other examiner • Police report of current conviction or prior related sex offense conviction • Psychosexual evaluation supervision conditions • Safety plans • Most recent treatment progress report, if requested • Any recent violation reports 3) Specific Issue: Random tests administered to address one specific issue and/or to confirm the crime of conviction. PO will provide the necessary information related to specific issue polygraphs.
