l. Contractor must have been in the janitorial business for at least two (2) years and employees must be supervised by someone with at least two (2) years' experience. Each employee shall have at least six months experience as a janitorial employee or be in a formal training program. Work history and capability will be determined by information provided on Vendor Questionnaire. 2. The Contractor must conduct background checks on all employees who will be used on this property and copies of the background checks must be available upon demand for Contract Monitors. The Contractor will not assign any employee to this contract that is known to the Contractor through background checks or any other source, to be a threat to the safety and security of the facility's employees, clients, or property. The Department of Mental Health reserves the right to interview, test, accept or reject any of Contractor's employees who may be assigned to the facility under contract. Should the facility reject an employee of the Contractor, it is the Contractor's responsibility to replace that employee in a time frame that does not interfere with the daily requirements of the contract. Contractor shall supply contact information to the Contract Monitor for all personnel engaged in work at the facilities on this contract. This list must be updated any time there is a change in personnel prior to a new employee working at the facility. 3. Contractor and employees will be required to sign a "statement of confidentiality". 4. Any calls to Contractor must be verbally responded to within one (l) hour. Contractor will be given a key to facility and security system access code if appropriate. Workers are always required to wear picture ID badges with company name or logo on them while on facility property. Contractor will be responsible for any loss associated with the loss or misuse of keys, including but not limited to replacement of door locks. Contractor's employees shall not leave keys in doors or admit anyone into the building that is not a designated employee of the Contractor. All doors that were locked upon entry will be immediately locked again. Contractor's employees shall not open drawers, file cabinets or use any telephone except public pay phones unless given specific approval by the Contract Monitor.
