Bar code AND tax receipt number required on front of white and yellow copies. Text to be pre-printed on front and back of form as follows: Front of the MVR-3E • Microprint displays vertically on the front left margin on all three copies. To read in two microprint lines: Void if Copied. State of Maine. • Bottom left corner on white and yellow only to have style 3 of 9 barcode and Tax Receipt number below barcode. Barcode measurements should be 1 3/8” wide x 3/16” high and the barcode color must be bright black laser printer barcode. The tax receipt numbers to be printed just under the barcode. • The bottom of the barcode should be located seven (7) text lines or 9/16” from perforation at bottom of each part • The start of the barcode should be three (3) text lines or 5/16” from left edge of paper • The Tax Receipt Numbers are consecutive numbers that start at 30873001 and continue through and including 31273000. • Bottom left corner on all three copies the form number and revision date in the following format: MVR-3E Rev 08/2016 • Just after the form number and date revision on the same line display, APPLICATION on the white copy and REGISTRATION on the yellow copy, TAX OFFICE on the pink copy. Reverse of the MVR-3E • White copy will display as shown in MVR-3E attachment. • Yellow copy will display as shown in MVR-3E attachment.
