A. This project is supplying and constructing an asphalt paving overlay at Michigan Avenue Elementary School, 188 Michigan Ave. School Rd., Cleveland Tennessee 37312 B. The proposal must include a schedule of values which are tabulated based on the areas identified in Attachment C, and noted below, all in accordance with the scope of work as noted below a. Area 1: Rear Entrance Driveway to Southeast Corner of building Area 2: Driveway & Parking area at South End of Building Area 3: Front Driveway connecting to North End of Michigan Avenue School Road. b. Asphalt Paving & Associated Scopes: Overlay of existing pavement. Place 2” of 64/22 Grade E Hot Mix Asphalt. Apply & Tack Coat to existing surface for adhesion (.03-.05 gal/sy). Restripe any existing pavement markings that are affected during the overlay process. Pavement markings shall be applied with latex traffic-grade paint. C. Listed below are additional calcifications and exclusions – including, but not limited to: • All work shall comply with all applicable codes, regulations, and/or local standards imposed by the local utility, city, county, and state. It is the contractor’s responsibility that all the construction be in accordance with Bradley County and TDOT Standard Details and specifications. • Contractor shall comply with all pertinent provisions of the “Manual of Accident Prevention in Construction” issued by AGC America, Inc. and the safety and health regulations for construction, as issued by the U.S. Department of Labor
