1. Contractor will have an active license issued by the Kansas Dental Board, and immediately report to Larned State Hospital (LSH) any changes to the aforementioned license. 2. Contractor will pass all security clearance checks prior to implementation of services. 3. Contractor will have malpractice insurance in place prior to implementation of services and will immediately report to LSH any changes in coverage. 4. Contractor will provide general dentistry services for patients/residents of LSH, including assessment and plan of care, fillings, crowns, bridge, root canal treatment, extractions, dentures, dental cleaning, bonding, oral and maxillofacial treatments, and other such procedures as privileged. 5. Contractor will provide no more than 24 hours of services per week in LSH' s Dental Clinic as needed. Any changes to specific dates and times for providing this service shall be established through agreement between the Contractor and LSH Ancillary Services Coordinator. 6. Contractor will place an order for dental supplies through the Purchasing office two weeks prior to the date dental supplies are needed. 7. Contractor will contact the Purchasing office immediately when dental equipment needs to be maintained and/or repaired. 8. Contractor will regularly review LSH Policies and provide practice standard updates to ensure compliance with all applicable regulatory/accrediting agencies including TJC, Medicare, and the Kansas Dental Board. Contractor may provide suggestions for quality improvement for continued survey readiness. 9. Contractor will assign to LSH the right to collect from third party payers for services performed for LSH patients under this contract. 10. Contractor will submit an invoice monthly to the Hospital's Medical Director's office by the 10th of each Month.
