The selected not-for-profit organization will carry out the following functions in the implementation of this new project duties: • Receive tenant applications • Review applications and supporting documentation for eligibility determination. Supporting documentation will consist of a copy of the lease for the property the tenant is being forced to vacate, photo identification, proof of income documentation, a copy of the condemnation notice posted, etc. • Confirm the size of apartment/unit the impacted tenant is being forced to vacate, specifically the number of bedrooms in the unit. The payment provided to impacted tenants is directly related to the HUD Fair Market Rate for the size of apartment being vacated. • Verify that the tenant’s current address is in fact condemned for occupancy due to documented building code violations as defined by the City Building Division. • Approve or deny the tenant’s application for assistance. • Distribute a one - time payment to the impacted tenant (or new landlord upon request by the Tenant) in a timely manner so that the tenant can in fact be approved for their new unit. • When applicable, communicate with prospective new landlords to confirm that the applicant is being considered for and has been approved to sign a lease for the new unit. Obtain a copy of the new written lease document. • When applicable, secure documentation that the approve applicant has been provided with a signed new lease for the new rental unit. • Maintain proper records of eligible tenants assisted, payments made, eligibility documentation reviewed and other accounting practices for monitoring and audit purposes.
