The City of Phoenix seeks to contract with a qualified vendor to provide COBRA Administrative Services to ensure the City’s compliance with COBRA regulations and to provide the following services, but not limited to: 4.1 Notifications 4.1.1 Initial Notice to newly eligible employees and eligible dependents. 4.1.2 Continuation election notices, Qualifying Event Notices. 4.1.3 Termination of COBRA benefits Notices. 4.1.4 60-day Notice of the end of Election Period 4.1.5 Open Enrollment Notices. 4.1.6 Delinquent Payment Notices. 4.1.7 45-day Notice of termination for non-payment of premiums. 4.1.8 Premium billing notifications and Notice of premium rate change. 4.1.9 Cancellation Notice/Termination Notice, regardless of voluntary or involuntary. 4.1.10 COBRA notices by first class mail and retain documentation of certificate of mailing to verify letter sent as required. 4.1.11 Non-sufficient funds notice The Contractor shall provide a draft copy of each notice for the City’s review and feedback prior to any City approved distribution. 4.2 Enrollment, Premium Collections, Processing and Billing 4.2.1 Accurate and timely COBRA administration, including premium collection and remittance. 4.2.2 Tracking of Election timelines 4.2.3 Collect and Process election forms. 4.2.4 Provide COBRA members monthly payment coupons. 4.2.5 Collect and record monthly COBRA premiums received from QBs...
