• FHW43 - Monthly Billing Coupons. • Quantity: 600M. • 28#, Eccentric Z-fold, Pressure Seal Form. • Size: 8-1/2" x 14". • To be printed 1/1, blue ink on front and blue ink security block out on back. • Forms to have .375" margins (critically required), 5 full-width horizontal perfs, 5 partialwidth horizontal perfs, and 1 partial vertical perf. • Boxes to contain 1200 forms in shrink-wrapped packages of 400, cardboard top and bottom each ream, 3 reams per box. • Forms must be able to run on Canon IR125 High Speed Printer and a Formax 2050 Sealer. • Sample supplied is actual form (see scanned b&w copy attached for reference only). • Successful vendor to contact agency for sample upon award. • Vendor will be required to supply 1,000 Test Forms prior to printing order (included in total price). • DELIVERY: Agency will request a shipment of 2 pallets as needed (approximately every 2 to 3 months). Currently a pallet contains 80 boxes of 1200 forms (96M). Vendor is to store forms and ship upon request. First shipment is needed as soon as possible. Vendor to coordinate first delivery, as well as all subsequent deliveries, with Paulette McKee 207-215-2745. Loading dock available. Lift gate truck may be required, depending of mode of transportation. • Vendor is to invoice for each delivery. • The cost of shipping and storage are to be included in the price • Overrun not to exceed 3%.
