Target Population: The target population of this RFI are all youth and their families, inclusive of gender, race and ethnicity, LGBTQ youth, youth with disabilities, or mental health issues, etc., from ages eight up to eighteen (8 up to 18) who are at-risk for any involvement with the juvenile justice system and are at high-risk for continued involvement. Proposed Services: Wraparound supportive services include family-centered, strength-based planning process, community collaboration, comprehensive assessments, trauma-informed care, mental health services, academic support, mentorship, systems, resources navigating, social support and community integration. The services should demonstrate the needs tailored to the youth and their family. Proposed Goals: Enhance family relationships. Increase relationships with peers and adults. Decrease risky behavior (anti-social behavior). Increase social competence. Increase school attendance and performance. Increase connectedness to community. Increase cultural awareness, skill building and pride. Promote self-efficacy, empowerment and self-esteem, inclusive practices to enhance acceptance of transition goals back into community.
