• The Enhanced Coaching Services Staff will meet with the Individual at least on a weekly basis or more, for the first four calendar weeks after the initial one-on-one meeting to develop the plan and start coaching. The plan will be completed and made available to the Department, as well as to the Individual, within the first four calendar weeks after the initial one-on-one meeting. • The coaching support needed to provide skill-building Services will include, job skill training, personal financial management, transportation services, retention of housing services, appointment scheduling and organizational tips, and other Services as identified, to establish wrap around service supports within the community, available adaptations to help with day-today functioning and retention of community providers, shopping, appointments, and other needs as identified, to preserve and enhance health, safety, and housing. • Local Office Case Managers will be invited to participate in the initial coaching session under the plan.
