A. Assist QACDOH with on site phlebotomy services including, but not limited to the following: a. The lab will be a physical site located in close proximity to Queen Anne’s County or neighboring counties OR is a mobile laboratory and is easily accessible for all Wellness Program patients who require in person venipuncture lab draws. The QACDOH will complete the lab order form for the requested services to send with the patient. Lab results will be provided to the QACDOH. b. The lab will employ experienced and certified phlebotomists, who will perform phlebotomy services that meet the standards of care c. The lab will have safe and hygienic testing procedures d. The lab will provide comprehensive testing capabilities Including, but not limited to, the following: i. 83036- Hemoglobin; Glycosylated (A1C) ii. 80061- Lipid Panel iii. 85025- Complete Blood Count (CBC) with Differential iv. 84443- Thyroid- Stimulating Hormone (TSH) v. 84075- Alkaline Phosphatase (Total) vi. 84439- Thyroxine (T4), Free, Direct vii. 80048- Basic Metabolic Panel viii. 80076- Hepatic Function Panel ix. 82043: 82570- Albumin/Creatinine Ratio, Random Urine x. 81001- Urinalysis, Complete with Microscopic Evaluation e. The lab will provide accurate and reliable testing methodologies f. The lab will provide access to a user friendly and secure online platform for ordering, tracking, and managing test orders and results
