1. The contracted wellness services shall be provided by a wellness onsite Registered Nurse (RN) and Specialty Nurse. The County is not looking for an onsite clinic model nor a single medical provider or doctor’s office to provide the services requested. 2. Screening and Wellness Consultation for Chronic Disease (Diabetes, Hypertension, High Cholesterol) 3. The Medical Director shall have the clinical oversight responsibilities of the nurse day-to-day operations at the worksite. The Medical Director shall ensure that the nurses are appropriately licensed, adhere to appropriate regulatory guidelines including HIPAA, and ensure that the nurse recommendations are consistent with approved medical guidelines. 4. Quarterly Onsite Screenings at County Departments/Divisions 5. The Contractor shall assist with organizing and participating in two (2) large annual events each year: Wellbeing Summit and Wellness Expo.
