At a minimum, the Offeror shall provide the following Services at the Agency’s location or an agreed upon location: i. Exposure Control Plan - Offeror shall assist the Agency in the development and review of the Agency’s exposure control plan. The Offeror shall provide annual review of the exposure control plan and provide recommended changes to the Agency. ii. Bloodborne and Airborne Pathogen Training - The Offeror shall provide airborne pathogen training for personnel that may be exposed to airborne pathogens, including Tuberculosis and will ensure training and information that meet the OSHA requirements as stated in 29 CFR 1910.1030 (g)(2). Additionally, the Offeror shall include airborne pathogen training for personnel that may be exposed to airborne pathogens, including Tuberculosis. iii. Vaccination and Examination Service - Offeror shall provide Agency with mutually agreed upon on-site vaccination and personnel’s annual physical examinations. 1. Scheduling Requirements - The Offeror shall have the scheduling flexibility and capacity to provide twenty to sixty (20 - 60) requested examinations within fourteen (14) calendar days following Offeror’s confirmation of Agency request for service. 2. Examination Results - Offeror shall provide a pass/fail result to agency within twenty-one days of examination. 3. Physicals - Upon request, Offeror shall provide new hire/annual physicals. iv. Medical Services - Offeror, upon request shall provide medical services that may include, but not limited to, Hepatitis B, Hepatitis A, tuberculosis (TB) testing, behavioral health services, laboratory services, and appropriate HIPPA required documentation and maintenance of records. Offeror shall provide additional medical services as follows: 1. Assist in developing forms, processes and procedures related to agency medical needs, these processes and procedures shall be in place prior to any release of any HIPPA protected medical documents. 2. Consultation and/or review of pre-hire medical questions that arise. 3. Consultation and/or review of fitness for duty and/or return to work questions due to such incidents as a fail of fit testing that arise and will provide a re-entry plan to return employee to their position...
