1. The overall task and responsibility of the provider of this level of housing shall include, but are not limited to, acquiring, developing, owning, and managing housing suitable for consumers; seeking various funding sources to finance the homes; constructing and renovating new housing projects; and providing to the DIVISION, property analysis and evaluation reports. 2. Ensure participation in the State and City’s Continuum of Care and the submission of annual U.S. Department of Housing & Urban Development (“HUD”) applications for various levels of housing (pending funding availability) in coordination and collaboration with the DIVISION. 3. Applicants are encouraged to propose and describe an innovative “mainstreaming” program approach that offers consumers empowerment and/or training opportunities, i.e., transitional employment positions, and full and part time employment, or other types of job training within the organization. 4. Incorporate recovery as a personal process of empowerment in overcoming the negative impact of a psychiatric disability and substance abuser disorder, and instilling hope and maximum independence despite continued presence of illness. 5. Provide eight (8) hours weekly staffing coverage. Additional staff coverage shall be determined by the needs of the consumer(s) in the home. 6. Ensure a maximum staff to consumer ratio of 1:8 in a home and 1:10 staff to consumer ratio if the service is in an apartment building. Changes to these ratios shall be approved by the DIVISION. 7. Ensure a maximum of two (2) consumers per room. DIVISION approval is required for the addition of new homes and for homes with more than six (6) beds. 8. Provide services in a manner consistent with the Comprehensive, Continuous, Integrated System of Care (CCISC) model of care, provided in Section 5, Attachment F. Address the needs of consumers with cooccurring substance disorders as outlined in each consumer’s MRP. 9. Have a policy that emphasizes a welcoming, empathic and integrated approach to working with individuals with co-occurring substance and mental illness. 10. Provide a safe and decent environment for consumers to live. All housing sites must meet guidelines outlined in the AMHD Housing Quality Standards. All sites are subject to inspection on a schedule to be determined by the DIVISION. Provide the DIVISION a plan of correction with specific timelines as required by the DIVISION for correction in areas found to be non-acceptable during the DIVISION’S housing inspections.
