Description of Services The Department of Rehabilitation (DOR) is seeking a qualified subject matter expert (SME) to develop and implement provider transformation model strategies for at least ten 14c certificate-holding entities. The Contractor will provide assessment (including summary reports and recommendations), model development and implementation that includes project plan development, implementation facilitation, training and technical assistance to at least ten California 14c certificate holding entities who have demonstrated a willingness and the capacity to engage in and transform to a CIE service delivery and/or business model. A description of each activity the contractor will provide is detailed below. Phase I – Assessment and Recommendations (months 1-6) Contractor, San Diego State University (SDSU) & DOR will • Develop, administer and analyze surveys and interviews of at least ten California 14c certificate holding entities to determine business operations, including capacity, and that include the following elements: o Communication with and support from the California SWTCIE Project Director and Principal Investigators along with key partners and stakeholders o Financial/Cost Assessment o Administration and supervisory support for transformation o Ability to adapt and implement CIE service delivery and/or business models o Ability to administer CIE service delivery programs o Ability to track and report performance o Ability to expand partnerships • Provide summary reports for each of the 14c certificate-holding entities with recommended transformation strategies to include Customized Employment, as appropriate. Phase II – Development of model transformation to CIE strategies (months 7-12) • Develop ten plans that include model transformation strategies consistent with the Department of Labor (DOL)/Office of Disability and Employment Policy (ODEP) and the Association of People Supporting Employment First (APSE) Provider Transformation principles, that incorporate DOL/ODEP resources and that address the following elements: o Vision and Mission Alignment to CIE o Funding diversification o Stakeholder identification and engagement o Program and staff development o Building sustainability Phase III – Model Implementation with Training and Technical Assistance (months 13-36) • Assist in the development of project plans for each provider o Set appropriate and achievable milestones o Identify potential barriers to implementation • Facilitate the implementation of the model provider transformation strategies in partnership with DOR and SDSU o Track appropriate milestones o Provide information with regular status reports o Communicate and work with entities to overcome barriers to implementation • Provide technical assistance and training during the implementation of the model provider transformation strategy • Assist in the facilitation of a Community of Practice for providers transforming.
