1.1.1.SCOPE AND BACKGROUND: Through this Request for Proposal (RFP) the Agency of Human Services (hereinafter the “State”) is seeking to establish contracts with one or more companies that can provide technical assistance to AHS and the Green Mountain Care Board (GMCB) in the design and implementation of the federal Centers for Medicare and Medicaid Services (CMS) “States Advancing All-Payer Health Equity Approaches and Development (AHEAD)” Model in Vermont. Technical Assistance will include supporting State efforts to: A. Assess and select statewide quality and equity measures, hospital measures, and primary care quality measures in accordance with AHEAD Model guidelines from CMS (see Appendix X of the AHEAD Notice of Funding Opportunity); and compiling existing data to assess and select measures. B. Work with the GMCB to design a statewide monitoring framework and measures for hospital global budgets to monitor access to services and other potential unintended consequences. C. Develop Statewide Quality and Equity Targets. D. Develop a Statewide Health Equity Plan.
